Jack, with the help of Sun and Kate, frantically treats Boone’s injuries. He sends Kate to the beach to get some supplies from Sawyer’s stash. Boone’s lung collapses but Jack manages to put a tube in to help him breath. Sawyer quickly gives up his stash of alcohol and offers to go with Kate. She politely tells him there is already “too many cooks”. Jack is still tending to Boone and is clearly overtaxed by the injuries. He goes to take a break at Sun’s insistence.
Sun brings back a stick and Jack dismisses it as an herbal remedy. In fact, the stick is for Boone to bite down on as Jack sets Boone's broken leg. It is a gruesome sound as the leg goes back into place. I say gruesome sound but in fact this episode is comprised of several very gruesome images and ideas. We see Hurley outside the medical area holding his ears so he can’t hear Boone’s screams. Meanwhile, Kate falls in the jungle on the way back, breaks some of the bottle, and clearly says “Fuck” under he breath. After which Kate finds Claire in the jungle clearly in labor. Kate tries to help Claire, but she tells Kate to leave her alone. Kate yells for help and Jin, who must have super hearing, hears Kate screaming over the noise of his hammering and the waves crashing onto the beach. Kate tells Jin to go get Jack and gives him the backpack to take with him.
Back in the cave Boone is muttering under his breath. Jack is trying to figure out how to use bamboo to give Boone a blood transfusion but first he needs to know Boone’s blood type. He mutters “A negative”. Jack tells Sun to get Charlie to ask everyone their blood type. Sun gets a sea urchin spine that can be used for a needle. Charlie manages to get four people who know their blood type but have no matches. Jack is close with his blood type O Negative but even though it is close it could still send Boone into anaphylactic shock.
In this episode’s flash-backs Jack is seen helping Scut Farkus from A Christmas Story (that’s his name, it took me forever to know that as I thought it was Scott; nope, it’s Scut) tie his bow tie. Here his name is Marc, and he is Jack’s best friend and best man as we see Jack is about to get married. Later, at the wedding rehearsal dinner, Sarah gives a speech thanking Jack for fixing her and for giving her the ability to dance at her wedding. That night, Jack and Sarah play Heart and Soul on the piano together. Jack tells her he still needs to write his vows. Jack seems to be looking for someone and Sarah reassures Jack that “He’s gonna come.”. Then she tells him she can’t wait to marry him and leaves.
Jack (who looks like warmed up shit) is giving Boone a blood trans-fusion with Sun’s help as Jin makes it back to the caves. Jin tells them (including Charlie) that Claire is in labor. Jack asks a bunch of medical questions and sends Charlie and Jin back and gives instructions to give to Kate to help Claire give birth as Kate is going to have to deliver the baby. Meanwhile, Claire thinks her contractions have stopped but then her water breaks. Boone begins to become more coherent, and he tells Jack how he really got hurt. He also mentions the hatch and tells Jack that Locke told him not to tell then Boone passes out from the pain again.Elsewhere on the island, Sayid has made a nice romantic rendezvous on the beach with Shannon. She tells Sayid about Boone being her stepbrother and that Boone is in love with her. She tells Sayid that she wants to take things slow with him. He accepts it and tells her he had no expectations about their “date”. Just hopes. Back in the jungle Charlie tells Kate what Jack said about Kate having to deliver the baby. Claire says she is scared, and Kate says she is as well. Jack and Sun realize the transfusion isn’t helping and Jack says that Boone didn’t get these injuries from a fall. Boone has compartment syndrome, and the blood is pooling in his leg. Sun tells Jack he has given Boone enough blood and removes the needle from Jack’s arm. Jack leaves the medical tent looking even worse than before. Hurley tells Jack he’s “looking kinda goth”. Jack sends Hurley off to find Michael and bring him back to the caves.
Christian Shephard finally shows up and joins Jack with a little foot soak in the hotel pool. Looks like Christian was the “He” who had not shown up yet. Jack is still not able to write his vows. In fact, Jack has doubts about even marrying Sarah at all now. Christian tells Jack he just isn’t good at letting go.
Back at the caves Jack is rigging up a device to use to amputate Boone’s leg as at this point, the leg can’t be saved. The leg is “dead” Jack says. He also tells Michael that Locke lied about how Boone got hurt. Claire has stopped trying to have the baby and refuses to push. She thinks the baby knows she didn’t want it and is going to hate her. Kate reminds her that Claire wants the baby now and it knows that too. Claire finally starts to push as Jin and Charlie wait nearby. As that is going on in the jungle, Boone begins coughing up blood and Jack says it’s time to take the leg. Sun tells Jack that he can’t save Boone. Jack yells back, “Don’t tell me what I can’t do!”. We hear that a lot in this show. As they prepare to cut the leg off using the plane’s cargo door as a guillotine, Jack tells Michael he doesn’t have to be there for it. Michael tells Jack that if he needs him to just call for him.
We go back to Jack’s wedding to Sarah, and he wasn’t able to write anything for his vows. Jack then speaks from the heart and tells Sarah he didn’t “fix her” but she in fact “fixed him”.
Right before Jack removes Boone’s leg, he regains consciousness, and Boone tells Jack to let him go. He tells Jack he knows he made a promise, but he is letting Jack off the hook. Something Rose also told Jack back on the beach right after the crash. As Claire gives birth to her baby Boone dies. His last words are “Tell Shannon I…”; which would have ended with “…love her.”, is the best guess. On the beach we see Claire present her baby to the rest of the survivors. In the distance Sayid and Shannon return from their night away together. Jack goes to meet them to tell them what happened. Jack has a hard time telling her and Shannon is devastated. She goes to the caves and cries over Boone’s body. Kate asks Jack if he wants to talk about Boone dying. Jack tells Kate Boone didn’t die but that he was murdered. He leaves and tells Kate he is going to go find Locke.
